REGISTRATION

PRUSSIAN TRAVEL SOCCER REGISTRATION

May 10, 2008

The following items are required for Prussians travel soccer registration:

• a wallet size photo (please write name and team, if known, on back)

• a check in the amount of $125 payable to KOPSC

• a copy of a State Birth Certificate (not Hospital Certificate)

• a completed US Soccer Player Registration Form or EPYSA RG-6 Form (see link below)

• a completed KOPSC Medical Release Form (see link below)

KOPSC MEDICAL RELEASE FORM (Boys and Girls)

Please contact your coach from last season regarding the submission of registration items. The list of coaches and their email addresses can be found in the link to the right.

Also, all coaches, please use the link below to print out your Risk Assessment Form for submission to US Soccer.

Fall 2008 Soccer

Boys and Girls ages 5 through 9

Dates: September 13 to October 25

Time: Ages 5 & 6 - 10:00 - 10:50am

Ages 7, 8 & 9 - 11:00am - 12:00pm

Location: Upper Merion High School Soccer Fields

Cost is $65.00 per player - Checks made payable to King of Prussia Soccer Club

Danny Bevilacqua Scholarship

Congratulations to Carolyn Kurz, Emily Hoover and Kevin Reifsnyder on being awarded the 2008 Danny Bevilacqua Scholarship. Each person was awarded $500 to take with them to college this fall. Good luck in school and keep up the good work.
